When you first telephone our office, you will be welcomed by one of our receptionists, who will begin gathering information about the nature of your concerns. They will ask for your address, so they may forward you a questionnaire that will be reviewed by our physician in preparation for your visit. You will also be asked to obtain medical records from any doctor that you have seen for fertility treatment as well as any records from recent routine care. We recommend that you make a copy of your records to keep for yourself before you send a copy to our center for our doctor to review. The questionnaire and medical records must be received in our office before an initial visit may be scheduled. You will also be asked to provide a copy of both partner's insurance cards (front and back) and insurance benefit booklets as well as a copy of each insurance company's third party claim form. Please note this also applies to any prescription benefit plan cards and formularies that you may have. These items, including the insurance benefit booklets, can be obtained at your employer's human resources office. These are requested so that we may assist you in maximizing the use of your insurance benefits. We will also need a copy of your hysterosalpingogram (HSG x-ray) films, if you have had this procedure. These are best obtained directly from the hospital or imaging center where the procedure was performed. If possible, a copy of the films (rather than the originals) would be best to obtain. Please have them sent directly to our office or bring them in with you at the time of your visit. Prior to your visit, you will be contacted regarding our special educational video disc (DVD), that covers important information about reproductive physiology and infertility testing and treatment procedures. Viewing this DVD will allow our physician and you to have a meaningful and productive initial consultation. For your initial visit, you should plan on spending the entire morning or afternoon at ACIRM. Most of your time will be spent in private consultation with your physician, wherein all of your questions will be answered and a personalized plan of care will be established. This is one of the special aspects that set ACIRM apart from other infertility and reproductive endocrine practices.
